The 28-year-old, who was the Sri Lankan hero in their famous one-wicket win over the Proteas in the first Test in Durban last month, injured his left hamstring during Sunday's third ODI, which was also played at Kingsmead.
Sri Lankan Cricket announced the news via their Twitter account on Monday.
The injury happened while Sri Lanka were fielding and Perera was sent for scans. He did not bat during Sri Lanka's unsuccessful run chase.
The tweet added that Sri Lanka would not be flying in a replacement for Perera.
The 4th ODI takes place in Port Elizabeth on Wednesday and Sri Lanka, 3-0 down, will be looking for their first win of the series.
